Allow trialist to choose a sponsor form a list

Description

When submitting a new trial, the trialist should be able to choose the name of a sponsoring organization or person from a list to avoid retyping. This should generate a copy of the sponsor record to be linked to the current trial.

The list of institutions and contacts is per user, that is, a user can only see the list of institutions that he has previously used in the current or a previous submission.
